Notice of Privacy Practices

Our Responsibilities
Helping Hands Child Care is required by law to:
• Maintain the privacy and security of your protected health information (PHI).
• Provide you with this Notice of our legal duties and privacy practices.
• Follow the terms of the Notice currently in effect.
• Notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
How We May Use and Disclose Health Information
We may use and share your child’s health information in the following ways:
• Treatment: To provide, coordinate, or manage ABA therapy services with other providers involved in your child’s care.
• Payment: To bill and obtain payment from health plans, Medicaid, or other payers.
• Health Care Operations: For quality assessment, staff training, licensing, accreditation, or audits.
We may also use and share information when required or permitted by law, including:
• Public health and safety issues.
• Reporting abuse, neglect, or domestic violence.
• Responding to legal requests or court orders.
• Assisting with law enforcement investigations.
Your Rights
You have the right to:
• Access: See or get a copy of your child’s health record, usually within 30 days of your request.
• Correct: Ask us to correct information you believe is inaccurate or incomplete.
• Confidential Communications: Request that we contact you in a specific way.
• Restrictions: Request we limit how we use or share your child’s information.
• Accounting of Disclosures: Request a list of times we have shared your child’s information for reasons other than treatment, payment, or operations.
• Copy of This Notice: Request a paper or electronic copy of this Notice at any time.
• File a Complaint: If you feel your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health & Human Services (HHS). We will not retaliate against you for filing a complaint.
Your Choices
You have the right to decide how we share information in situations such as:
• Sharing with family, friends, or caregivers involved in your child’s care.
• Using information for marketing or fundraising purposes.
• Releasing psychotherapy notes (if applicable).
We will never share your child’s information without written permission for:
• Sale of information.
• Most uses of psychotherapy notes.
• Marketing communications.
